The 11 On-Page Issues That Actually Move an SEO Health Score

Published 2026-07-18 · 8 min read · by the isaeo team

In short

Eleven on-page issues drive most of a page's SEO health score. Three are high severity — a missing title, a noindex tag and a missing H1. Four are medium: missing meta description, thin content, multiple H1s and no canonical. The rest are low-cost polish. A page returning 4xx or 5xx scores zero regardless.

How the score works

Every page starts at 100 and loses points per issue, weighted by severity. The site's health score is the mean of its page scores — so one catastrophic page can't sink an otherwise healthy site, and a thousand small faults can't hide behind one perfect homepage.

One rule overrides everything: a page returning 4xx or 5xx scores zero. A broken page has no SEO value regardless of how good its meta tags are.

High severity

These three cost the most because each one can remove a page from search entirely, or leave the engine guessing what it's about.

Medium severity

Low severity

Low severity does not mean ignore. It means don't fix these before the ones above them.

What to fix first

The efficient order isn't "worst issue first" — it's "worst issue affecting the most pages first". An issue's real cost is severity multiplied by page count. One missing title on an orphan page matters far less than a missing canonical across 400 product URLs.

  1. Anything returning 4xx/5xx — those pages score zero.
  2. Unintended noindex tags.
  3. Missing titles and H1s, highest-traffic templates first.
  4. Template-level medium issues — fixing one template fixes hundreds of pages.
  5. Everything else.

isaeo's prioritised fix list does this ranking automatically, sorting issues by severity times affected pages, so the top of the list is genuinely the highest-leverage work. Once you've fixed a batch, re-crawl and confirm the score moved — plenty of "fixes" quietly don't ship.

Frequently asked questions

What is a good SEO health score?

Treat 80+ as healthy, 50-80 as needing work and under 50 as urgent. Because the score is a mean of per-page scores, a moderate site-wide number often hides a small group of very broken pages — check the distribution, not just the average.

Which SEO issues matter most?

Missing titles, unintended noindex tags and missing H1 headings carry the highest severity, because each can remove a page from search or leave engines unable to determine its topic. Pages returning 4xx or 5xx errors score zero outright.

How often should I run an SEO audit?

Monthly for most sites, weekly for sites that publish or deploy frequently. The value is in the comparison between crawls, not any single snapshot.
